Strangvac® – a modern
multi-component vaccine

Strangvac® is developed to protect against the contagious and painful bacterial disease strangles. It is based on recombinant proteins (harmless parts of the bacterium) instead of live or completely inactivated streptococcal bacteria, Streptococcus equi. Strangvac® contains recombinant protein antigens from Streptococcus equi. They are not live and cannot spread to other animals. For approved indication and full product information, see FASS Vet.

On August 16, 2021, the EU Commission granted a marketing authorization for Strangvac®, making it the first Swedish animal health vaccine to be approved via EMA's central procedure, which covers all member states of the European Union. Strangvac® has also been granted marketing authorization in the United Kingdom.

Clinical studies are currently being conducted in the USA, and we are seeking a distributor for the American market who is ready for registration and launch there.

Innovative technology creates new opportunities
The vaccine is developed with innovative technology based on fused recombinant proteins. The research underpinning the vaccine's technical platform has been conducted in collaboration with researchers at Karolinska Institutet and the Swedish University of Agricultural Sciences, SLU.

The marketing authorization is based on the results of a series of clinical trials that studied Strangvac®'s safety, immune response, efficacy, and ability to differentiate vaccinated from infected horses (DIVA compatibility).

Strangles – one of the oldest equine diseases

The contagious respiratory disease strangles is one of the oldest known diseases in horses. The disease affects horses worldwide, and in Sweden, one to two strangles outbreaks occur every week, often linked to the import of infected horses.

Horses can be carriers for a very long time, even after symptoms have disappeared. A Swedish research study at SVA (H1147203) showed that as many as 92% of examined horses remained persistent carriers after a standard three-week isolation. The same study showed that 67% remained carriers even after six weeks of isolation if not properly tested.

Increase in strangles globally
Globally, strangles is classified as one of the most common and widespread infectious diseases in horses, and the number of strangles cases is increasing. The disease is endemic in almost the entire world and constitutes a major economic and animal welfare problem.

In Sweden, around 60 outbreaks (stables – not individuals) are reported annually, and in the UK, approximately 1,000. In Sweden, strangles is a notifiable disease to the Swedish Board of Agriculture – however, this differs between countries globally.

In addition to the suffering of affected horses, an outbreak entails significant economic consequences. An affected stable or riding school needs to take extensive restrictive measures to prevent further infection, which also includes closing operations for sanitation and keeping horses in quarantine.

STRANGVAC® & STRANGLES

This is strangles

Symptoms and treatment
Classic symptoms of strangles include high fever, yellow purulent nasal discharge, and swollen, painful lymph nodes in the head and neck that can develop into abscesses. The disease is caused by streptococcal bacteria and varies greatly in severity between individuals.

Treatment is assessed by a veterinarian based on the disease phase and the horse's condition. Antibiotics are used in specific cases, while supportive care, isolation, and infection control measures are often central.

Mature abscesses under the jaw may need to be opened and drained by a veterinarian to relieve pressure.

Sequelae and complications
Mortality from strangles is usually below 2% in uncomplicated cases but can rise to 8–10% if serious complications occur. Sequelae and complications are relatively common, affecting up to 20% of infected horses.

» Bastard strangles: The bacteria spread via blood or lymph and form abscesses in internal organs such as the lungs, liver, spleen, or brain. Bastard strangles is one of the most common causes of death or euthanasia.

» Anasarca (Purpura haemorrhagica): A severe, immune-mediated inflammation of the blood vessels (vasculitis). It causes severe, painful swelling in the legs and head, as well as bleeding in the mucous membranes. The condition has high mortality if not treated promptly.

» Guttural pouch empyema and chondroids
(concretions): Pus accumulates in the horse's guttural pouches and can dry into hard "stones" (chondroids). This makes the horse a chronic, silent carrier that may require surgery.

» Airway obstruction: Extremely swollen lymph nodes can press on the trachea and pharynx. This leads to acute respiratory distress (hence the English name strangles).

» Muscle inflammation and emaciation: Some horses suffer from immune-mediated muscle atrophy or myocarditis.

Piggivac™
– vaccine candidate against S. suis

Piggivac™ – or INV274 as the project name – is our development project for a vaccine against Streptococcus suis infection in pigs. The vaccine candidate is based on recombinant proteins, and the next development step includes preparations for GMP manufacturing and registration-enabling clinical studies.

Streptococcus suis is an important global endemic swine pathogen causing meningitis, sepsis, arthritis, and endocarditis in weaned piglets between 4 and 10 weeks of age. Mortality can be as high as 20% on some farms, and the total cost of S. suis infection in pigs is estimated at more than 250 million euros annually in Europe alone. Reports from the field (Vaccine, article no. 127077) show an increasing number of S. suis infections. This has created a great need for a functional vaccine, as commercial alternatives are lacking.

S. suis serotype 2 is the globally most common cause of severe disease in piglets. Serotype 9 is, currently, less common than serotype 2 but is increasingly a cause of severe disease in pigs in Europe. Protection against multiple S. suis serotypes has been requested by the pig farming industry for many years.

Intervacc is the first and so far only company in the world to demonstrate – in two separate studies – that piglets are protected against the virulent and severely pathogenic S. suis sequence types 1 and 16 (serotype 2 and 9 respectively) by vaccinating sows that transfer immunity to the piglets.

The results represent an important step in the continued development of the vaccine candidate, as the strains used in the studies are not closely related and the vaccine was only administered to pregnant sows with transfer of immunity to the piglets.

Reduced need for antibiotics and improved profitability for pig farmers
It is estimated that S. suis accounts for one third of the total antibiotic use in weaned pigs. A safe and effective vaccine against S. suis would reduce the need for antibiotics and the global threat from antibiotic resistance, playing a significant role in improving animal welfare and enhancing profitability in the pig farming industry.

The INV274 (Piggivac™) project has, in fierce competition, been awarded funding via the Eurostars-2 program. The project phase for which Intervacc, together with Moredun Scientific in Edinburgh, has now been granted funding includes resources to carry out several important development steps, including preclinical efficacy studies in piglets, antibody analyses, and vaccination of pregnant sows.

PIGGIVAC™ & S. SUIS-INFECTION

This is S. suis infection

Symptoms and treatment
In pigs, Streptococcus suis
primarily causes meningitis, arthritis, and sudden deaths. It most often affects weaned piglets up to 10 weeks of age.

In humans, the infection causes high fever, neck stiffness, severe headache, confusion, and hearing loss.

The infection is acutely treated with intravenous antibiotics, usually penicillin or ampicillin. In large outbreaks, antibiotics can be administered via drinking water or feed.

TECHNOLOGY

Icons showing a verification mark, a DNA molecule, and a microscope

The fundamentals of our technology

The technology with fused recombinant proteins for the modern vaccines developed by Intervacc is based on nearly 20 years of research at Karolinska Institutet and the Swedish University of Agricultural Sciences, SLU.

The technology is complex, but here is a brief account of its basic principles, which form the platform for the vaccines Intervacc develops.

For a bacterium to be infectious, it must be able to attach to a tissue surface. This occurs through surface-localized bacterial proteins with a specific binding capacity (here called adhesins) to, for example, collagen, fibrin(ogen), fibronectin, and several other tissue structures.

Antibodies have two functions
The principle of Intervacc's vaccine development is to identify such surface-localized bacterial adhesins and produce them on a large scale using recombinant technology.

When used as vaccine components, antibodies are formed that have two main functions:
1) During an infection, the antibodies can bind to the adhesins, thereby hindering the bacterium's adhesion to the tissue. This blocks the infection process.
2) Antibodies that bind to surface-localized structures, such as adhesins, send a signal to white blood cells to internalize and kill the bacterium.

The proteins that constitute adhesins have, among other things, been identified through bioinformatic analysis of the DNA sequence from Streptococcus equi and can be compared with previously known virulence factors from, among others, Streptococcus pyogenes, a human pathogen similar to the one causing strangles.

An illustration of how the vaccine works

Other proteins have also been identified whose function is important for the infection process, for example by impairing the horse's immune response. Antibodies against these have a neutralizing effect and deprive the bacterium of this “weapon”. This impairs the bacteria's immune defense capability.

Fusions at the DNA level
The effect of a vaccine increases significantly when several components are included in the vaccine. To still keep production costs down, Intervacc has created fusions at the DNA level so that several proteins are included in the same polypeptide. For example, Strangvac® contains three polypeptides, but these originate from eight different proteins.

Strains of S. equi isolated from strangles outbreaks in different parts of the world have a very high degree of similarity to each other. This means that only one version of the vaccine is needed to provide global coverage.

The same strategy for vaccine development has been chosen for vaccine candidates against Streptococcus suis and against Staphylococcus aureus.

Strangvac® vs. conventional vaccines
Conventional vaccines against bacterial infections generally consist of live attenuated or killed bacteria or of subcomponents, such as capsular polysaccharides and toxoids. When an attenuated bacterium is used as a vaccine, a subclinical infection occurs, often with mild or no symptoms, but antibodies are formed that have a protective capacity. However, there is a risk that the mild infection, caused by the vaccine strain, develops into a serious infection. Furthermore, it is possible that the generated immune response targets proteins that are less useful and therefore do not provide strong protective immunity.

When using conventional bacteria-containing vaccines against strangles, it has been reported that horses suffered from strangles infection instead of being protected. These strangles vaccines are used only to a limited extent. Intervacc has therefore opted out of this vaccination method.

Overall, Intervacc has the technology, expertise, and strategic collaborations that are required to take a project from research stage to finished product.
